In surgical slide conference today we discussed osteomas, and how the bones of the skull are a common site for these benign bone tumors.

AMISH KIDS AND THE CONVENTION

Clicking through Ann Althouse's site and saw that I did almost the exact same thing she did last night: watched parts of the Democratic Convention and that Amish kids reality show while flipping channels. I had some of the same reactions. Like, could they find any more limited and narrow examples of 'city kids' to unleash on the poor Amish kids? (And by limited and narrow I mean totally predjudiced against the Amish. I like that they tried to find a heterogeneous group of people). You had the following: a young gay man who immediately made fun of the Amish kids, a vegan who thought cows come from outer-space, a guy from Boston who was a jerk (how likely is that?), a hottie who said he was 'both persian and white' and an African American woman who was actually pretty nice after getting over her initial shock. So, two of the five were ok, but their first reactions were still embarrassing. I guess good manners are not a requirement for, well, anything anymore. How can this show be real? It's like some gruesome psych experiment that they did in the old days (you know the one where kids keep clicking some button until they think they kill someone by clicking some button. Oh, you know which one). One Amish kid tried to show the city kids a toy he had carved out of wood and he was so shy and clueless I could hardly bear to watch. Ditto for the convention.

I watched parts of Edwards speech. Nice wife introduced him and cute kids hugged him at the end. In between, he smiled and sparkled and got that misty, far-away look in in eyes, like, look how far I've come, just look! Which normally would be quite touching with the looking lovingly at the parents and all, except that he made millions of dollars from suing docs, and as a doc, I must say that I object! Er, sorry. Never coulda handled law school by the way, have you seen what they have to read? Good God. Anyway, did anyone else cringe at the use of Hope? What is it with hope this and hope that? Hope is on the way, help is on the way, I come from a town called Hope and boy, do I hope I win this thing. I mean we win this thing. Whatever. I'm sure the RNC will be equally painful. What kind of person wants to be a politician anyway?

I love my apartment on Lake Street with it's big picture window and views of the Sears Tower and Chicago skyline. The lights sparkle in the distance, rows and rows of lights against a dark sky. I am sitting on the couch which is covered in green cloth and is very comfortable. I have a book in front of me (I am not reading it) and a cup of tea is sitting on the table. The cup and saucer are white china with a pattern of blue flowers. They belong to my mother who has sent them to me. I am waiting for a visitor. When he arrives, with a knock at the door and covered in snowy clothes, I hardly know what to say. He, on the other hand, says plenty. "Do you have anything to eat? What, no, no not that. Let's go out. What are you reading? Oh my God, that book doesn't look promising. The woman on the cover looks depressed. Why are you sitting here drinking tea like an old lady, anyway?" We go across the street to an Italian restaurant. Or, should I say, "Italian" restaurant as my companion is Italian and hates the restaurant on site. But he orders gnocchi anyway, saying in his slightly raspy from smoking too many cigarettes voice, "This could be dangerous."

WE DISCUSSED LENTIGO

Maligna in conference the other day. Sir John Hutchison first described lentigo maligna in a paper titled "Senile freckle with deep staining - a superficial epithelioma of the cheek," Archives of Surgery, London, 1892, 3:159 (from the excellent website: Who Named It?). Also, I have adapted their medical disclaimer for my website. I'm sure they'd understand. Sad that we live in such a litiginous world, where we have to worry about these things...

I SPOKE ON THE PHONE

with R yesterday. I could hear baby K (who is now toddler K) screeching in the background. "Yeah, she does that now," he said. Also, "she says morning. It's so cute." And with a little prompting I hear a babyish voice say, clear as a bell and sweet as one too, "morning. Morning." Oh man. Do I love that kid.

Three main things need to go in the gross description of a pathology report, people. A description of the lesion, the size of the lesion and the location in relation to the specimen margin. Lesion, size, margin. Got it?

DIFFERENTIAL DIAGNOSIS OF PAGETOID CELLS IN THE EPIDERMIS

Paget's disease (extramammary and mammary), melanoma, pagetoid squamous cell carcinoma in situ, porocarcinoma (which theoretically arises from a different portion of the eccrine duct than extramammary Paget's disease), sebaceous carcinoma, rarely Merkel cell carcinoma and pagetoid reticulosis (Woringer-Kolopp disease - a T cell lymphoma of the skin). Can you think of any others to add?

Discussed this in surgical slide conference today and I have to say the resident did quite a nice job discussing the differential.
